Quality Reality Check

Navigating nursing home options in Iowa means understanding the quality landscape. With 390 facilities and 25,357 beds across the state, it's essential to look beyond the surface. Approximately 38% of Iowa's nursing homes carry a lower overall rating of 1 or 2 stars (80 facilities are 1-star, and 68 are 2-star). This percentage is slightly better than the national figure, where more than one-third of nursing homes are rated 1 or 2 stars. The average star rating in Iowa is 3.1 out of 5. When it comes to staffing, Iowa nursing homes average 0.73 RN hours per resident per day. This is above the new federal minimum standard of 0.55 RN hours per resident day. However, families should still watch out for critical issues like understaffing, especially since many rural Iowa facilities are limiting admissions due to a lack of workers. Serious deficiencies, including those related to infection control and potential abuse or neglect, are also critical red flags on inspection reports.

Will Medicaid Cover It?

Iowa Medicaid, also known as Title 19, can be a crucial resource for covering nursing home care, but eligibility has strict financial and medical requirements. For a single applicant in 2026, income must be under $2,982 per month and countable assets under $2,000. You must also require a Nursing Home Level of Care (NFLOC). For married couples where one spouse is applying, the applicant spouse still faces the $2,982 monthly income and $2,000 asset limit, but the non-applicant spouse may keep up to $162,660 in assets through the Community Spouse Resource Allowance. If income or assets exceed these limits, Iowa has "spend-down" provisions. This means you can spend excess funds on approved medical expenses, home modifications, prepaying funeral costs, or paying off debt to meet eligibility. Medical Assistance Income Trusts (MAITs), sometimes called Miller Trusts, can also help manage excess income. Be aware of the 60-month look-back period for asset transfers. The application process can be complex; you can apply online via the Iowa HHS Services Portal, by phone at 855-889-7985, or in person at a local HHS office. The Facility and Waiver Eligibility Team (FWET) processes these applications.

Finding the Right Facility

Medicare's Care Compare tool (medicare.gov/care-compare) is your go-to resource for researching Iowa facilities. It provides star ratings for health inspections, staffing, and quality measures, offering a comprehensive overview. Be mindful that many rural areas in Iowa are experiencing significant nursing home closures and bed shortages due to workforce challenges. This can make finding a facility challenging, sometimes requiring placement further from home. When reviewing facilities, always check the "Statement of Deficiencies" from the Iowa Department of Inspections and Appeals (DIA), which is publicly available on their website or at the facility itself. These inspection reports detail findings on cleanliness, safety (including fall risks and fire safety), medication management, staff qualifications and levels, and adherence to resident treatment plans. Pay close attention to any serious deficiencies that posed an immediate jeopardy to residents' health or safety, as well as infection-related issues.
